HIV/AIDS Case Management

Through the Effingham CARE Consortium, a federally funded program administered by HHS, any individual regardless of financial status with verified HIV or AIDS may receive Case Management Support Services, which provides information, referrals and assistance in making linkages with physicians and other social service resources in local communities. Services in the program address:

  • Health Care. Financial assistance is available to pay for HIV-related medical care, dental care and alternative therapy for individuals who do not have other forms of insurance and who meet income guidelines.
  • Housing. This service provides rental and utility payment assistance. Individuals must meet specific housing and income criteria to access these funds from the CARE Act and from Housing Opportunities for Persons Living with AIDS.
  • Mental Health/Substance Abuse Treatment. Expenses for this service are provided for individuals within the qualifying income range when no other forms of insurance are available.
  • Nutrition Assistance. Nutritional supplements or food vouchers are given to qualifying individuals.
  • Transportation. CARE offers gas vouchers and provides payment to transportation providers to transport clients to and from HIV-related appointments.
  • Legal Services. CARE provides legal services for cases related to a person’s HIV status. Services include the preparation of wills, trusts, powers of attorney, Do Not Resuscitate orders, permanency planning, etc.
  • Support Groups. Monthly support groups are provided to those with HIV and their families.
